Legal Name Change Form For Dennis Baxter-Gphangez Selcsum-Gnortz

Dennis submitted two name changes, but he said that it was accidental because he thought the first one hadn't come through. It was suspicious that so many people had changed their names in one day, and it was suspected that there was some forgery involved.
